Do you or any member of your household have COVID-19 or are you waiting for a COVID-19 PCR test result? (not a routine surveillance test result)
Are you required to self-isolate (including arrival from overseas)?
Do you have ANY of the following symptoms now, or in the last 14 days?
- Fever, acute cough or shortness of breath
- Muscle aches, loss of smell, sore throat
- Generally feeling unwell with no other likely diagnosis
- Do you have any other reason to think that you are at risk of having COVID-19?
If you have any of the symptoms mentioned above, please call the practice on 03 355 2348 or email us to email@example.com to reschedule your appointment. Please DO NOT come to the clinic.
Please, arrive as close as possible to your scheduled appointment
Come alone unless accompanying a patient who is a child or an elderly patient
Scan the QR tracer code on arrival at the door or sign in at the desk
Wear a mask at all times in the waiting room. If you have a mask exemption please wait in your car and call reception when you arrive
